A lead form (or instant form) opens inside Facebook, Instagram or TikTok when someone taps an ad. Fields such as name, phone and email are often pre-filled from the profile, so people can submit in a couple of taps.

Why it matters for clinics
Lead forms usually bring a low CPL, but intent tends to be lower than on website forms; people may even forget they filled one in. That's why reaching a form lead within minutes is critical. Leads shouldn't sit in the ad manager; they should land directly in the CRM with the right agent.
Example
Someone fills in the Instagram lead form on a hair transplant campaign. The lead lands in the panel with the campaign details and is assigned to the English-speaking team. Within minutes, an agent messages them on WhatsApp and sends the photo guide.
